Unable to get captions to appear in a list of figures in Microsoft Word

I've done everything I can think of, but my List of Figures only shows the Figure # and then the page number:

The first two figures for some reason work just fine, but the rest won't. I've right clicked the images, inserted a caption, but it just doesn't work. I desperately need help here, this is for my dissertation and I just can't make this work correctly.

Here is one that works:

And this one (and the following) do not work:

Your Figure # and description must be in the same line or at least paragraph. If there is a paragraph break after the Figure #, change it to a manual line break (Shift+Enter) and try again.

The Table of Figures is really a Table of Contents Field with special switches/parameters. Here are some references:

I have the same problem, but if I do a manual line break (shift + enter), the # of the table disappears, the word 'table' is still there. I would be very grateful for any help as it concerns my dissertation.

Did you look at the links, especially the first one I provided?

Could you attach a sample document with the problem (Use the paperclip). It should have at least one figure and caption with the problem and the Table of Figures. It need not be your entire dissertation.

Thank you for your fast answer! I actually tried to do it in a new word document but I could not replicate the problem. The manual line break worked out fine in the new document. I tried to figure out what the difference is between the two documents by clicking "edit field" and then StyleRef. Different styles were used, in the new document "caption" in the original it was called "1". However, when I tried do change it do "caption", the same problem came up.
