Certificate not available on Certificate Dashboard.

Hello Team,

I completed Azure Fundamental Certification but unable to see it from "View Certificate" option. From the "View Badges" option its only showing the previous MCP Badge i got on 2015.
However i received the Badge on the email i provided below.
Please help me out to get the certificate as I need to provide it at my work. Not sure about the details i am supposed to give so i will provide all the details i have so far below, please let me know if anything else is needed.
