You can use the Windows API ShellExecute(Ex) to invoke the URL.

I guess you're free to implement a help ID to url mapping however you want - you just have to do it yourself.
I gave up on context sensitive help many years ago - I just have a single html file; users can use the browser to search the file for what they're looking for.
In my application, for each dialog and propertypage context sensitive help id added. I have added new property sheet in my application I need to add context sensitive help id for my new propertysheet laso.
Have a look at this article, it explains the normal MFC context sensitive help framework. It should provide you with the background information you'll need to get started in implementing your own mechanism.
The default implementation for CPropertySheet Help is like this:

Notice that it gets the active page and invokes its help topic. Is your intention to not show the active page's help?
MAKEHM needs an ID, IDR or an IDD. CPropertySheet afaik doesn't have its own IDD. You could create a new command ID (say yourID) in your application, handle WM_COMMANDHELP in your property sheet and call AfxGetApp()->WinHelpInternal(lParam) where lParam is = HID_BASE_RESOURCE + yourID
