You can use the PowerShell script to get the permissions of a document library in a specific SharePoint Site and export it in .csv format . The script gets the permissions report for all objects of the list (such as folders or files). Please find it in attachment: 209745-permission-report.txt

Unfortunately, it does not include the date/time that the permission is assigned to the user.
