We would like to be able to access the FHIR SQL data directly for regular back end updates from our offsite database without using the FHIR api. How can we do this? Is there a schema we can reference?

Unfortunately, you can't access the SQL DB from the managed service for compliance reasons.

Thank you for your response. Is it possible to use our in-house sql database together with your fhir api in some un-managed way?
Also, could this be maintained directly through your common data model?
