Click Home > Common > Questionnaires > Questionnaire schedules. Click Setup details.

You can use this form to set up responders, questionnaires, and people being evaluated.

Task that uses this form

The following tables provide descriptions for the controls in this form.

Tab	Description
Responders	View a list of queries to select from to schedule a questionnaire.
Questionnaires	View a list of available questionnaires and select one or more to schedule.
Setup	Select a worker, contact, or applicant that the questionnaire is an evaluation of.

Field	Description
Query	Select a standard query of responders to the questionnaire from the list.
Questionnaires	Select one or more questionnaires to schedule.
Evaluation of	Select a person to evaluate. The Evaluation of field must be completed if the person who answers the questionnaire differs from the person who is making the evaluation.
