If you don't want to use ShellExView (which is a portable freeware), the only option is to edit the registry to find the context menu handler and the corresponding DLL file. If you're willing to continue further, first please create a System Restore Point.
WTF is Microsoft Designer and how do I unistall it?
I just right clicked and image and said "create with Designer", I found out it's some AI slop shit called Microsoft Designer. How do I unistall this shit? I didn't consent to this crap

From the Options menu, enable "Hide all Microsoft extensions".
Locate the Designer context menu entry from the list.
Select the entry and press Ctrl + C to copy the file info.
Post the info here.

- Also, is the program listed in Apps & Features?

Abra Configuración → aplicaciones. Vea si puede desinstalar "Microsoft Designer" de Aplicaciones y características.
Si no aparece en la lista, debería poder desinstalarlo mediante PowerShell.
Abra PowerShell de administración y ejecute estos comandos:
get-appxpackage -allusers *MicrosoftDesigner* | remove-appxpackage -allusers get-appxpackage -allusers *MicrosoftDesigner*Si el 2º comando muestra alguna salida, publique la captura de pantalla aquí.
