You just fill out the form using the email address you're trying to get back into, you know. Make sure to add another email where they can reach you with updates or whatever. Then, answer all those questions as best you can, like old passwords you remember or subject lines from emails you sent a while back. The thing is, the more stuff you get right, the better shot you've got at Microsoft actually verifying it's you and letting you in.
