It is possible that USB Flash drive is failing, try the following fixes, in both cases, make sure you are working on that USB drive . . .

Open Disk Management (accessible by right clicking your Start Button)
In the bottom pane of Disk Management, right click the partition on that drive and choose Delete Volume
Then on the resulting 'Unallocated Space, right click and choose New - Simple Volume

If that operation fails:
Click your Start Button, then just type cmd
On the resulting list, right click Command Prompt and choose 'Run as Administrator'
Run this command and hit Enter:
diskpart
Run this command and hit Enter:
list disk
(Make note of the disk number you want to convert and enter it into the next command - replace x)
Run this command and hit Enter:
select disk x
Run this command and hit Enter:
clean
Close Command Prompt and open Disk Management, please provide a screenshot of that Disk Management window . . .
