Windows updates issue

Hi All

On one of my windows 2012R2 server i am facing issue with updates. Updates are not getting installed but it shows updates are installed, server reboots and again updates are getting installed.
in control panel under view update history, i can see repetitive updates but they are not getting installed. Experts guide me on this

    How are you confirming? As an example for KB4601349 you could check the file versions listed here against your system. Your system should have at least the same or higher file versions.
