The database is very simple, barely has any data or any calls being sent to it. Seems like a near empty unused database should not even last less than half a month on a free plan. Am I doing something wrong? It seems there is some sort of 'health event updated' being ran every few hours in the activity log that's unpausing it and causing it to run down the vCore seconds.

I have the same problem. It eats something like 10% of my vCore seconds per day at the start of the month, so after 10 days it's down to zero, without me doing anything with it. It is useless.

If you are talking about Azure SQL database, it is "100,000 vCore seconds of SQL database serverless usage per month with 32 GB of storage"
