Microsoft Word Mail Merge Issue

I am trying to create a mail merge document using an existing list of information from Excel. When completeing the merge Word is converting dates into numerals despite having them formatted as dates in the Excel document.

I have tried a number of ways to remedy this issue without success. Can you offer any solutions?
