Sharepoint 2013 Designer Workflow Not Running on Created Item sporadically

I have a Sharepoint Designer 2013 workflow that started acting up about 6 months ago. Once every say 3 months, the workflow just does NOT run. The workflow is set to run on item creation. I have checked the audit logs and the workflow history. The audit log just shows the item being created but no workflow. The previous item that was created shows the workflow running successfully. The Workflow history rightly shows nothing for the problem item but does show rightly the workflow running on the previous item.

This doesn't happen regularly only say every 3 months. Any idea where to look?

    Hi @K R ,

    You could try to check whether the problem item is created by the system account. By default, SharePoint 2013 workflow cannot start automatically by System account(either on create or change).
