programming a patient list

Hi, I have a document where I add pt name, medical record number, DOB, diagnosis, location and to-do. I am currently adding them manually and deleting them manually. is there any way i can program it so i can just click an add button, or some sort of drop down so i can easily add, remove rows without the whole insert, format etc.

    The buttons are already in Excel. Customize the QAT and add the buttons to insert and delete row.
