Hi all,
Draw a WedgeRectCallout in PowerPoint and drag the handle to inside the rectangle. You see a full rectangle. Save the presentation.
Open the package and exchange the prstGeom with the corresponding custGeom from presetShapeDefinitions.xml from the OOXML standard. Open the changed file in PowerPoint. You see a notch in the rectangle.

The red shape is the shape with custGeom, the gray shape in the middle has prstGeom, the right object is a screenshot of the gray shape in edit mode. I have sent the document itself to dochelp@microsoft.com.
Such shape with a handle inside the rectangle is not a user error but such shape is part of SmartArt "Interconnected Block Process".

This is a documented bug. Not sure when the fix will arrive in presetShapeDefinitions.xml, but the fix is in the Word docx attachment on https://mailman.vse.cz/pipermail/sc34wg4/2018-November/004555.html. There are fixes for circularArrow, wedgeRectCallout, wedgeRoundRectCallout, and wedgeEllipseCallout in that document.

thank you for the link to the document. The proposed changes to wedgeRectCallout work for me. I know, there exists some more corrections to the presetShapeDefinitions.xml. Could you bring a corrected version of the file to ISO? Shouldn't that be possible without the full process needed for a new version of the standard?
@Regina Henschel , good to hear this worked for you.
Regarding other changes to the standard, I'm just a developer like you, so I don't have any ability to influence how updates/changes to the standard get published.
There used to be something called the OpenXML Implementation Guide which listed out clarifications and bug fixes that seemed to have been updated on a more regular basis. It's got a different name now, I don't remember what it is, but it is super useful.
