Bot Icon show up incorrectly

I developed a custom app, and upload package from Team admin, so other user can see it.

The issue is when other member add this app/bot, it show bot icon incorrectly in some place, but icons show correct in other place, see the chart:

some icons show default icons, we expect it show my own icon.

why this issue happen? I already logout /login many time.
