How Do I get email addresses within a survey?

I've created a survey, but cannot figure out how to have the email address show up. How do I make sure that data is captured?

If you create a poll, it is anonymous. If you create a Form and add it as a tab in Microsoft Teams, the response will include the display name of user who fill out the form.
