ISSUE WITH BOOKING AZ-104 EXAM

Hi,

Im encountering a problem with submitting infos for my residence, email and phone number for booking my AZ-104 exam, it's the first page that pops up when you click "Take Exam" in the AZ-104 page, what could be the issue? thank you.
