Microsoft Forms - Structure and Formating

I was wondering if anybody had a quick solution on how to format a specific questionnaire on Microsoft Forms. Below is an example of what I am trying to do. The survey would have several categories and people would then rate those categories from 1-5 on several variables that go horizontally.

Hi mirzoyana,

We are sorry that there is no correct formatting in Forms will meet your requirement.

However, if you want to collect responses form many users using the formatting and structure your provided. You can use Excel workbook as a workaround. You can create a workbook and share it to other users or store it in SharePoint Online site, user can access it and leave their values. Once it stores in a cloud place like SharePoint online and OneDrive, this file will be a shared file and multiple users can together.
