Adding forms in PowerPoint

When inserting a form or quiz in PPT,  I get the error "You don't have permission to view this form". I can create the form and quiz but it doesn't show up in the presentation.

    thank you for your response. I am signed in with a Business account and only use my business email address.

    Hi IzaneCloete-Hamilton,

    Thank you for bringing this issue to our attention. I hope you're having a good day despite the trouble you're experiencing with your PowerPoint application. In line with the error message, you need to make sure that the email account that is signed in to your PowerPoint app should be the same of what you used to create the Form. Could you confirm this?

    Additionally, could you please verify which Office 365 version are you subscribed to? Is it Office 365 for Business or Home/Personal?

    Hi Izane,

    Sorry for the late response. You are the creator of this form, in theory you should be able to view/edit it. And seems your forms works fine on http://forms.microsoft.com/

    To narrow down the issue I'd ask a few questions:

    1. Though the pane loads your forms, please double confirm whether your logged in account is correct or switched by another one. In PowerPoint app, click File>Account to find it.
    2. Check if this issue also happens to a newly created form.
    3. Update your PowerPoint app to the latest version to see if there's any difference.(File > Account > Update now)
