Thanks for posting back and sharing the forms.
To protect your private information, I will have to remove these links. Based on my test, here is a screenshot of when I answered all questions and tried to submit the form. The Submit button was grayed out and the message said "Duplicate this form to use as your own". It means you shared the form with us as a template, not sharing the form with me to collect responses.
Based on the situation, I would like to confirm if you also used the same way to share the form with others as a template. If so, it is expected behavior that users can't submit the form successfully on their side. You should click "Collect responses" to share the form as shown in the screenshot below.
If you originally meant to share the link with me to collaborate and view results, please note that sharing the form as a template won't make me able to view the Responses of the form.
Normally, you should be able to view responses in real-time when using the correct shared link. Please try point 1 in my previous reply. Also, what if you access and submit the form yourself (access the link from "Collect responses"), will you see the response?
