Error Code: 500121 indicates Authentication failed during strong authentication request. The user didn't complete the MFA prompt or they may have decided not to authenticate or timed out while doing other work, or has an issue with their authentication setup.
Check if the account MFA is blocked.
Entra ID > Security > Multifactor authentication > Block/unblock users.
If this doesn't work out, try to require re-register the user's MFA registration. This will delete the user's MFA and prompt them to register policy required MFA methods.
