MIM error "Exception has been thrown by the target of an invocation"

When I click on 'Refresh Interfaces' on an ECMA2 MA, I get the error "Exception has been thrown by the target of an invocation". I also notice that when running a preview sync, that as soon as it gets to an flow where there's an advanced flow rule I get an error. a generic Error: unexpected-error is thrown.

Here's another clue: This server is in a DEV environment for a client, I figured this server was hosed, so I found another server that was working (non-prod) - I installed Visual Studio. I made one change to the code on an MA rules extension, complied the DLL (pointing directly to the extensions folder) and it started having the exact same problem as the earlier server. I believe it might be having an issue reading the extensions folder now, but I have no evidence of that. Anyone seen this before?
