Hello! My forms are not loading on MS Forms. I have deadlines coming up. The last response I received was over 8 hours ago. Just wondering if anyone is having loading issues with Forms.

For the situation you encountered, we deeply understand the inconvenience caused and apologize for it.

We recently received similar issue in this community, all the Microsoft Forms are not accessible. I would like to make a summary on this issue to find the information easily. This will also help other community members to find much easier of they come across the same situation:

Issue Description:

Microsoft Forms are not accessible. Sorry, something went wrong, please try again. Error code: 1003/640.

Environment:

Office 365 for business.

Suggestion:

It might be a server-side issue and needs to be investigated from back-end side. In this situation, we sincerely recommend you contact your Microsoft 365 administrator, follow steps in this article and create a support ticket to contact Online Support. The support team over there has higher permission than us, they can remotely help you and collect logs. They can also involve more resource to investigate the issue from back end. This would be the most efficient way in handling this case for you.
