SharePoint List items gets modified automatically and user in modified by column doesn't know that happened.

Hi Team,

I have SharePoint list and facing the issue like some of the list items gets modified and even user doesn't done it manually. when we go through logs , got below information. There is outlook instance i can see attached.
Kindly let me know how to resolve this one as its happening again and again.

1. What is the version of SharePoint are you currently using?
2. Where did you get the log? Have you checked the SharePoint ULS log and Windows event viewer?
3. How many lists are experiencing this issue? If only one list has this problem, have you checked the workflow settings?
4. Is this column being modified by the same user or multiple users?
