Authenticator not working

The user recently reset his password. After doing so, he was unable to log in with the authenticator app. When attempting to authenticate using OTP, he is prompted to enter a CAPTCHA. After entering the CAPTCHA, the following error appears. We also tried using the authenticator app, but when scanning the QR code, it shows another error. I have reset his password and re-registered MFA, but the issue persists. Could you please assist me with this?
