It let's me deny but if I pick a number (right or wrong) and hit approve then I get this error. I then have to put my password in and then deny the request and then approve the new request that will have the 5 digit code instead. This is on the new fold4 on android.

I had the same error after migrating from a Samsung S21 ultra to a Samsung S22 ultra.
I had to uninstall and reinstall the authenticator app and then sign in with my account again and now it is working

I also backed up the Authenticator App data to the Cloud in Authenticator App Settings, uninstalled the Authenticator App, restarted my Galaxy S20, installed the App afresh and started to recover from backup as written at the bottom of the presented list. When prompted to verify my identity via my email I failed to receive the code on my mobile and my PC which was signed into the same Microsoft account and email. Fortunately, I keep a second Galaxy S8 with the Authenticator App (and other crucial Apps) periodically updated (as a fall back device) and when starting that, the code appeared there which I cancelled as I'd started over again to recover from backup on the S20. I then used the Authenticator App on the S8 to complete the verification of the S20. So a little disturbing about getting the code!
