Confused with IF IFS IF/AND Formula/Function

Hi There,

I know this is super basic and should not be so hard to understand.  I am trying to created a permutation on this form that raw data is usually imported on. I think I would want to use IF/AND but I am not sure.  I have attached a sample form and the columns I have highlighted in yellow would be the data I would need to calculate for a final outcome dependent on the answers to those columns.  I have also listed the items that I would choose from but have not put together all of the possible outcomes.

Any help or advise would be more than appreciated.
