Can I save an MS Form template to a sharepoint

Hi, 

I have an MS form that I have created and I would like to share this form as a template to a SharePoint that I am a member of, so that colleague may use the template.  Is this possible and if how so, how is it done?

    Hi Renee,

    Thank you for posting your question.

    Within SharePoint site, I am afraid we cannot save a Microsoft Form template in SharePoint document library or service, like a Word document template to reuse, as the forms can be only stored and accessed from Microsoft Forms page.

    However, as your goal is to let you team members reuse the form you created, if your SharePoint site has connected to a Microsoft 365 group, you can create a form template in Microsoft Forms under this group.

    For your information:

    Create a group form or quiz

    In this scenario, when your group members need to use this template, they can go to Microsoft Forms under the Group forms to Copy the template form as a new form for using.
