Mail Merge Template

I know I asked a similar question a bit ago, but I really need detailed instructions on how to set up a template in Word for mail merge.  I have a spread sheet already set up as input.  I've read the documents on mail merge, but it's still not making sense to me.  Sorry to be a pain, but I really need the help.
