Microsoft Forms Upload Error

I am having issues with a form I recently created to allow users to upload files. When testing it myself and with certain users, it works, however it has been failing for some users within my organization. A rather non descriptive error appears as below:

I've inspected the console logs for what the requests are returning when this error appears and the following appears:

It seems to indicate a permissions issue. I've tried giving those users form ownership, access to all folders the form creates, etc but to no avail. Some specifications about the setup is below

  1. All users are on the organization network, and the form is currently set to allow responses for users within my organization 
  2. All file sizes are below the 100mb limit I've placed
  3. All files being uploaded are the accepted format types (All Excel files). To double check I've uploaded the same files that these users could not upload from my account and it worked.
  4. I've tried providing users ownership access to the forms and all folders and subfolders it links to
  5. I've tried deleting the question and recreating it
  6. I've tried recreating a brand new form
  7. My users are all trying to upload through the shared form link on a Chrome browser, although other browsers have been tested too including Edge, Firefox, IE and Edge built on chromium.
  8. All users are using Windows 10, on standardized fleets of notebooks. Problem appears to be account-centric, as logging in on another computer yielded the same issues.
  9. No remote working software like Citrix or firewalls are present

The problem is quite perplexing and is preventing the full rollout of our Office 365 suite across the organization, so some timely advice would be appreciated. Thanks.

    Apologies for the late reply Chitrahaas - it's been hectic just getting things to work. I've tried all your suggestions above, and the issue persists. We ended up having to perform a workaround. It appears that personal forms can be sent out and files can be uploaded just fine. the issue is when the forms are owned by a Sharepoint Group instead. Even members of that group are unable to upload the files. It's quite strange and we haven't been able to work this out. What we ended up doing was to create personal forms and have our flows interface with those instead, which is not an ideal solution, as these forms can get deactivated when a user account is deactivated upon leaving the organization, etc.

    Hi SJS,

    Sorry for the inconvenience caused at your end and appreciate for your efforts taken to troubleshoot the problem encountering.

    Generally, when you add a File Upload question in the Microsoft Forms, a new folder will be created in the user’s OneDrive who created the Form to store the files uploaded by the users in the organization. Moreover, the default permission for the Folder will be “People in Contoso with the link can upload only

    So, every user in the organization have access to upload the files to the folder created by File Upload unless you share the form with link “Specific people in my organization can respond”.

    In this situation, since you have tried different browsers, new form with File Upload question, and problem only with specific users, I would like to request you try the following:

    a. Ask the problematic users to switch the network connection and check whether they can upload the files without any problem.

    b. Give the Folder link to the problematic users directly and check whether they can upload the files without any problem.

    To give folder link: Open your OneDrive> Apps> Microsoft Forms> Open the Form with File Upload question> Select Folder with Question> Open the details pane> Manage access> Copy the link> Share with users.

    In addition, if the other users create form with File Upload question, can the problematic users upload the files with out any problem?
