Please help, whenever I try to add new slide, in the layout I only have title slide in the layout, Why is this happening?

Is this also the case in a new PPT file? If no, maybe there is only a title layout left in the master. Go to View - Master and see if the other layouts have been deleted.

This is happening when I open a new ppt file, and once I close the file, unsave it and open it again, the issue gets resolved, but not all the time
I also tried the reset option next to it, it works in a few instances but not everytime

If the thumbnails appear in the master but not in edit mode there could be a problem with the video driver that needs to be updated.
I remember thumbnails not showing up some years ago, the problem was usually the hardware acceleration. If you go to File - options - advanced - display section you can uncheck disable hardware acceleration...
Add-ins can cause some trouble, so you can disable them temporarily to see if it helps.
