For some reason in the table of contents below, the right align page numbers isn't an option anymore and I'm not sure how to get it back on.

The issue with the table of contents was that a \p " " switch had been added to the TOC field and that switch causes a single space to be inserted between the text of the table of contents entry and the page number, rather than the default tab space which is required for the page numbers to be aligned at the tab stop.

All of the tab stops show up in the correct position, but the page numbers don't like up with it

If you update the table of contents, does that fix the issue?

If not, see Suzanne's reply about the exact positions.

If none of these suggestions helps, accept Doug's offer and send him the document, so that he can take a closer look.

If you send me a copy of the document, referencing this thread in the covering email message, I will investigate the issue.

In the TOC, click in an affected entry and drag the right-aligned tab stop (that is intended for the page number) to the right so that it becomes positioned in the correct position.

It appears that the right aligned tab stops have gone missing from the TOC # styles. The simplest thing may be to delete the Table of Contents and then create a new one. Failing that, you will need to modify the TOC # styles which can be done by clicking on the Modify button in the Table of Contents dialog shown in your screen shot.
