We noticed that you have tried the suggestion to remove this pen. However, this pen moved back to 1^st^ position.
If my understanding is right, for the situation you encountered, we want to confirm with you if you are using OneNote for windows 10.
If so, we’d suggest you go to Windows Settings > Apps > OneNote for Windows 10 > Advanced options > click Reset and repair, and it will help reset all setting in your OneNote for windows 10.

- Could you provide the screenshot of the pen in your OneNote?
- May I know if you are using OneNote for Windows 10 or OneNote 2016?
