How do I turn the result of a Microsoft Form into a Power Point Slide, one result per page?

I want to create a dynamically populated Slide in PowerPoint that is populated from the results in a Form.

Each Form result will create its own slide.

A button or trigger would be nice.

I have done this before on Google Sheets / Google Forms, but trying to figure out a Microsoft version.

Any help would be awesome.

    An excel form ... do you mean an Excel worksheet? Where's it stored?

    Look, the more detail you can provide, the more likely it is that someone will be able to help you. Some of us know VBA and PowerPoint very well, but we're not experts at mind reading or teeth pulling. You need to help us help you or work it out on your own. Your call.

    I have no idea what I am doing. The ultimate goal is to have a user fill out a form. The form is to allow people to submit Veterans in their life for a Memorial Wall.

    I'd like for them just to answer a few questions, and upload an image and we could auto-generate the Memorial Wall in PowerPoint.

    It's not clear whether you want them to fill out a VBA form in a PowerPoint PPTM file or a web form or something else entirely.

    There are commercial add-ins that would let you start with an Excel spreadsheet with all of the data plus the name of the image file and generate the slides for you. Would that solve at least part of the problem?
