Here are two ways that can achieve your needs (for the new form with a live Excel workbook).
Method 1:
- Go to https://forms.office.com/ and log into your business account.
- Under "My groups", click "Show more", find the Microsoft 365 group associated with the SharePoint site, click the group, then click "New Group Form". The group form is associated with a live Excel workbook by default.
Method 2:
- Go to the related SharePoint site and go to the library.
- Create a new Excel workbook and open it in the browser.
- Click the "Insert" tab, Forms, New Form. Please note that the form name will be the same as the workbook name.
