A web-based tool in Microsoft 365 that enables users to quickly create surveys, quizzes, polls, and feedback forms.
Apparently, you cannot add fields to capture demographics without being part of the question set, but my workaround was to separate the demographics from the quiz by using sections. Although the name and the title/location fields have numbers, they are separate from the quiz that will start at question 3 on the next page. Obviously, there are no correct answers for these two questions, but since the quiz is in a different section, I can obtain a correct score for the actual questions to be scored.
