How to add the respondent email and Name to Microsoft Forms

Apparently, you cannot add fields to capture demographics without being part of the question set, but my workaround was to separate the demographics from the quiz by using sections. Although the name and the title/location fields have numbers, they are separate from the quiz that will start at question 3 on the next page. Obviously, there are no correct answers for these two questions, but since the quiz is in a different section, I can obtain a correct score for the actual questions to be scored.

Yes, same here! How do I just add a name field to the quiz?

I would also like to know how to simply add a field ("Name") to the quiz form without it being a question so i can pair responses with the person who filled it out.

(I want anyone with link to be able to access the quiz without signing in, so i need the setting "Anyone can respond".)
