MS Form Upload document is consistently failing.

Users are trying to upload files to our MS Form, but they are all getting a failure message. All files are the correct format and many we checked met the required size.

    I’m sorry you’re having a problem but based on your description we can work together to resolve it.

    For the situation you encountered, we deeply understand the inconvenience caused and apologize for it.

    As per your description, since you mentioned "All files are the correct format and many we checked met the required size"

    I suspect, when you create an upload question, the file will be stored on OneDrive or SharePoint (for Group forms). When users upload file in Microsoft Form, they will be stored in a specific folder (Apps/Microsoft Forms). 

    If you delete one of the above folders, there is no storage location anymore and users will get error message "Failed to Upload your file, when they try to upload a file in Microsoft Form question.

    Once the folder is deleted, you have to delete your 'upload question' and generate a new one for having a sync with the right folder. You may try to just delete the items(upload file  quection type) itself and then again create an upload file  quection type n Microsoft Form and then let users upload file to test.

    Meanwhile, if possible, could you please confirm does this issue occur only a specific Microsoft Form? it will help for us to narrow down your issue.
