I need to access the themes but there is no design tab. Searched how to find it and it says go to "options" under the file tab but there is no "options'.

The online and mobile Word programs do not use the Design tab.
The desktop programs/applications do.
The design feature in the Online version is not a tab and is very different from the one on the desktop programs. It is a button on the Home tab and is more like the designer feature in the desktop PowerPoint.

If you are using the online, web browser, version of Word, there is no Design tab, as Hans and Charles have pointed out.
You can choose to display design suggestions if you click Home > Designer > Designer.
P.S. Only Microsoft can answer "why" questions. Posters here are not in a decision making capacity with Microsoft. However, it is a reasonable guess that the online version of Word will continue to be more basic compared to the versions that we actually have to pay for.
