PPE/Uniform Inventory Tracker Sheet

Hi,

I am looking to redesign our tracker sheet (it's included inside the cabinet for people to fill out), for any spare PPE/uniform that is kept it our cupboard.

This is purely to issue out any any spares, people take what they need and record on the sheet what they have taken and sign/initial it, we don't need anything complicated just something that is better than the below, otherwise people won't bother filling it in!

Can anyone come with something that is more suited to what we need.

    It's ok i've managed to come up with something that has separate sections on the same page for the excisting spares stock list count and recording what has been taken.

    Let me put it another way: imagine you had to take a tool in our company.
    Would you be able to fill out a form like yours? I tell you clearly: you are not.

    If I were in your company and take a T-shirt from the cabinet, I would expect a form that asks me:

    What did you take?
    How many did you take?
    Who are you?
    And maybe when did you take it?

    I don't know anything else beyond that. Therefore, this information is reasonable and IMHO completely sufficient.

    The order code is not what they write in that is already on the sheet, we just want a record of everything that has been taken out of the cupboard. We need to account for everything. I should have made clear, that the final 3 columns is what people actually write on the sheet.

    We have something very similar in our company for cutting tools. If I compare it with your form, IMHO you ask for far too much information.

    You already know exactly what was taken out via the "Order Code", so "Item Name" and "Size Taken Out" are superfluous.

    If "Initial's" makes it clear who took it out, why then do we need PPE/Corp? It would only make sense if that the user takes something out for someone else; I assume that the user belongs to PPE or Corp...

    "Qty Available In Cabinet" is wrong, nobody wants to count the current inventory, change it to "Qty taken".
    You can then make the appropriate deductions in your inventory system and then know how much inventory is still in stock.

    The date is also questionable, what is the use of this information? Who cares if I took a T-shirt out on Aug 15 or Sep 20... gone is gone.

    So all that remains is "Order Code", "User Name" and "Qty taken", nothing more is needed.
