Microsoft Forms Participant Information Sheet and Consent form

Hi,

I'm currently setting up my survey for my masters thesis and I need to add the participant information sheet and consent form. I need to add these before the actual survey, I know we can upload the participant information sheet as a file but as it is anonymous, it won't allow me to upload it. I've seen plenty of surveys on Microsoft Forms with this done but I can't find any video or instructions on how to do it.

I would really appreciate any feedback.

    Thanks for your reply to my query

    So, I tried the Section part to insert my participant information sheet but for some reason it wont let me add my whole participant information sheet. It is quite detailed so maybe that why, I even tried doing in manually rather than copying and pasting and it still wont let me finish adding the whole document. When I do add text question type it wants me enter an answer but I don't need to enter one.

    I'd appreciate if you could help me further

    Dear Susannah McD,

    Thank you for your update. I understand the frustration you're facing with the blank space appearing in the text questions.

    To resolve this, I recommend using the "Choice" question type instead of "Text" for the information sheet sections. You can format the text in the "Choice" question and leave the answer options blank.

    At last part, you can also add a simple option like "I have read the information."

    This should display the text without the unnecessary answer input box. The question will appear in the form as just a static text block, which seems to be what you’re aiming for.

    Alternatively, your idea with the Linktree QR code sounds like a creative workaround, especially if you want to provide separate access to the PDF document and the survey.

    Thanks for getting back to me

    I've tried breaking up the information sheet into smaller sections with the text option but when I preview it there's a blank space there where you insert an answer and I don't want that.

    I've seen many surveys done on forms which don't have this. My second option is a linktree QR code which will have two tabs one which is a pdf document of the information sheet and the other is the questionnaire.

    Let me know if there's any other way to do this

    Regarding your challenge, the text question type in Microsoft Forms has a character limit, which might be preventing you from adding the entire document. I recommend breaking up your participant information sheet into smaller sections and adding multiple "Text" question types for each section of the document.

    To address the issue where it asks for an answer: You can leave the answer field blank if you're only using the text question to display the document content. This way, the form will still accept the question without requiring an input. Simply enter the text in the “Question” field, and you should be able to proceed without needing to add an answer.

    If the document is still too lengthy or complex, another option would be to host the document on OneDrive or another cloud platform and include a link to it in your form, as the 2nd option I mentioned in my previous email.

    • If uploading files isn't an option, you can host the documents on a cloud storage service (like OneDrive or Google Drive) and provide a link to download them in the text section of your form. So participants can review it before proceeding with the survey.
