On Xbox, achievements must unlock reliably when the in‑game criteria are met. If they do not, it is treated as a defect in the title’s achievement implementation.
From the certification rules:
- An achievement that never unlocks after the criteria are met is a Critical issue: “An achievement is not unlocked when the criteria to unlock it has been met and does not unlock at a later stage in test; that is, it never unlocks.”
- If achievements fail after standby or network reconnection, that is also treated as a failure:
- “Achievements do not unlock after resuming from connected standby, and is still not awarded after terminating and relaunching the title.” (Critical)
- “Achievements do not unlock after reconnection to Xbox services.”
Because these behaviors are not expected, the permanent fix must come from the game developer or publisher via a title update. As a player, the recommended actions are:
- Make sure the console is online and signed into the correct Xbox profile when attempting the achievement.
- Fully quit and relaunch the game, then try the requirement again.
- Power cycle the console (shutdown, unplug for 30–60 seconds, plug back in, restart) and retry.
- Check for and install any game updates; developers often patch broken achievements because they violate certification requirements.
- If the achievement still does not unlock after repeating the requirement, report it directly to the game’s support channel or publisher so they can fix the achievement logic in an update.
If achievements only unlock after restarting the title or only sometimes unlock, that is still considered a defect (Minor/Very Minor) and should also be reported to the game’s support team.
