URL Rewrite Provider - Managed Type is blank

I am trying to use the URL Rewrite Db provider and following these steps: https://learn.microsoft.com/en-us/iis/extensions/url-rewrite-module/using-custom-rewrite-providers-with-url-rewrite-module#using-dbprovider

I have installed the URL Rewrite Module 2.1 on our IIS Server and URL Rewrite do show on IIS.

However, when adding a provider and the Managed Type field is blank.
