Lumia 640 Reset Protection recovery key

I've got five boxes on my screen that I have to put something in to be able to reset my phone. It's not a phone number, and it's not my account password or anything. This is what I have to enter:

_ _ _ _ _ - _ _ _ _ _

_ _ _ _ _ - _ _ _ _ _

         _ _ _ _ _

No idea what I've got to do. Help?

    the reset protection is really good in a case of theft but what the owner died of the , do we just throw the phones?

    i has lumia 640XL that requires reset protection recovery key and my sister is late now, do i just throw the phone?
