Constant crashing / bad_module_info.

I'm constantly crashing in games. This error code is from Warhammer: Vermantide 2.  About once every 10min. 

In event viewer i get this:

I also get Bad_Module_Info crashes with Apex Legends.

Does anyone know how to fix this?

    It happens regularly. Sometimes i can play for 15min but sometimes it crashes 3 times in 5min. I'm getting these crashes in Warhammer: Vermantide 2 and Apex Legends.

    I ran CKHDSK but it said that, Windows has scanned the file system and found no problems.

    No further action is required.

    The error log recommends CHKDSK.

    To run CKHDSK -

    Type cmd in Windows Cortana search

    Right click on cmd.exe > Run As Administrator

    Type following and Enter to fix problems on the C: drive while you're using it, and will use as much system resources as allowed so that it will run as quickly as possible. Say Y if it says that it will start CHKDSK at next restart

    chkdsk c: /r /scan /perf

    I found some logs at the application and system tab around the time of the crash.

    I am Vijay, an Independent Advisor. I am here to work with you on this problem.

    Post entries corresponding to this event and few minutes preceding this event in Event Logs.

    Windows Key+R > Type eventvwr and Enter > See if any log corresponding to your date and time is there (Look under all entries under Windows Logs such as System, Application, Security...) > If yes, right click on that log > Save Selected Event > Zip all event files which correspond to your event, upload them to Onedrive and share the link here.
