I had the same exact issue on a 1.5 year Lenovo All-In-One.  I suspect is caused by malware or malware removal software.

I contacted technical support at Lenovo who re-installed drivers and eventually sent a tech to replace speakers and mic.  My camera went out as well and was replaced.

None of these steps fixed these issues

I then did a factory reset using the Windows 10 boot options.  Hold the Shift key down while restarting the computer to access these options.  Then choose, 'Reset' option that saves your personal files.  Your user settings and post-factory programs will be removed, however.  Make sure to back up files just in case to another drive.

This reset fixed my audio issues.

Try this and let me know what happens.
