Azure Multi-Factor Authentication Server and Exchange 2016 OWA

I have Azure Multi-Factor Authentication Server deployed in my environment for RDS and VPN. This is an existing deployment and I know Microsoft does not allow new deployments of this. Nonetheless, I would like to use this MFA for Exchange 2016 on-premise OWA. I believe this should be fairly simply done using IIS authentication and probably forms-based. However, I am not sure exactly how to configure this. Has anyone done this? It looks like I need to know the variables used for username and password to setup forms-based authentication in the MFA server.

    But note, this is completely unsupported as far as I know. :(
