Let's remove paired devices, on your PC, go to Settings > Devices > Bluetooth & other devices, find your phone in the list of paired devices and click "Remove device". On your phone, go to Bluetooth settings and remove the PC from the list of paired devices.
Turn off Bluetooth on your PC and your phone, restart both devices.
Are the Bluetooth drivers up to date?
On PC, open "Device Manager" (right-click on the Start menu and select "Device Manager"), find "Bluetooth" in the list and expand it. Right-click each Bluetooth device listed and select "Update Driver", choose "Search automatically for updated driver software" and follow the instructions.
