The previous night I was using my PC. The next morning when I went to check E-mail I had the "Blue Screen" with the error "Your PC did not start correctly" with the options of "Restart or Advanced Options". After restarting I have the IO1 Initialization Failed. Found post on Microsoft without any success. I have attempted posted resolutions. Still getting the IO1 Initialization Failed. I cannot boot up in"SafeMode" or any of the other modes from BIOS boot up.

Hi, thank you for that information. In this case, we need to run a custom installation to fix the issue, However, the process will delete all apps but will retain all files on the PC, If you are okay with that, you may follow the steps provided below:

Since you have already downloaded Windows 10 on a USB, kindly follow the steps from number 3 to the last from this link:

Thoroughly research any product advertised on the site before you decide to download and install it.

Additional note: If the issue persists, I suggest contacting a local technician to physically check the device for any hardware-related issues.

just to let you know I am UTC -6

Got to the Windows Setup, Clicked on Repair PC, then Window Recovery Enviroment, I then click Windows 10. It attempted window repair. After about 3-5 minutes it came back and "Startup Repair couldn't repair your PC". Looking at E:WINDOWS\System32\Logfiles\Srt\SrtTrail.txt. file it has "Startup Repair has tried several times but still cannot determine the cause of the problem."

After that failed I attempted to follow the "Command Prompt" route.

At the first step of DISM /online /cleanup-image /restorehealth

    it showed version: 10.0.19041.3636

    Error: 50

DISM does not support servicing Windows PE with the ?online option.

The last entry for DISM.log file is dated 2025-04-17

Hi, thanks for reaching out. My name is Bernard, a Windows fan like you. I'll be happy to help you out today, my timezone is UTC +8.

I understand the issue you have, there is nothing to worry I am here to assist. This error means there is an issue with the input/output of the PC, in this case, I recommend using a Windows installation media in USB to repair the issue, to do that kindly follow the steps below:

Note: We need another PC and a USB drive (At least 8GB and make sure there are no files inside as it will reformat the USB) once you have that kindly follow the steps:

Step 1: Create Windows 10 installation Media

    Go to this link: https://www.microsoft.com/en-us/software-downlo...

Scroll down and click "Download Tool Now" then run the Media creation tool (the downloaded file)

If you agree to the license terms, select Accept.

On What do you want to do? page, select Create installation media for another PC and then select Next.

Select the language, edition, and architecture (64-bit or 32-bit) for Windows 10.

This table will help you decide which edition of Windows 10 you’ll choose:

On Select which media, you want to use click USB and save it to your USB drive.

Once done, do the repair kindly follow the steps below:

Insert the USB into the affected PC

Start the PC and if you see “Press any key to boot from DVD,” do so. If you don’t see the message, you might need to change the boot order in your computer's BIOS settings so that it first starts from the DVD or USB.

When you see the Install Windows page, tap or click Repair your computer to start the Windows Recovery Environment.

In the Windows Recovery Environment, on the Choose an option screen, tap or click Troubleshoot.

Tap or click, advanced options and then click on Startup repair.

Let me know how it goes and I hope that helps.
