To narrow down this one, I would confirm with you that:
Which version of Exchange on-premise that you used? If you are using Exchange 2010, please have a check about "DataImportTimeout" in "C:\Program FIles\Microsoft\Exchange Server\v14\ClientAccess\exchweb\ews"
If you aren't using Exchange 2010, I would suggest you trey to create a new migration endpoint, check whether there exist issue with it.

That suggestion is used to SMTP match Exchange on-premises and Exchange online mailbox. You need to check whether there exist duplicate mailbox on your Exchange on-premises and Exchange online.
If there exist duplicate mailbox, you need to get the GUID for Exchange on-premises mailbox from the step1, then connect to Exchange online set this value for the duplicate mailbox on Exchange online. (replace the UserName with duplicate mailbox email address)
If there doesn't exist duplicate mailbox, this suggestion will not suitable for you.

The solution is not the GUID. I am still researching for the solution..
